- Description: The primary objective of this study was to assess the pattern of change in neurocognitive performance for adolescent athletes on baseline measures of the Immediate Post Concussion Assessment and Cognitive Testing (ImPACT) test, over five consecutive years, with a view to providing an indication of the optimal interval for repeat baseline testing of high school athletes. Participants were non-clinical, predominantly South African high school athletes in the overall age range 13 to 18 years (N = 108), divided into five groups (Grades 8,9, 10, 11 and 12), and tested at five test intervals. Repeated-measures ANOVA analyses examined differences in score performance across the test intervals for each of the five composite scores of the ImPACT test (Verbal Memory, Visual Memory, Visual Motor Speed, Reaction Time, Impulse Control). For the Verbal Memory, Visual Memory, Visual Motor Speed and Reaction Time composites there were significant neurocognitive score changes between several test intervals. Taking these results into account, in conjunction with substantial variability in performance, it is concluded that there is a need for annual baseline testing throughout the high school years. The secondary objective was to generate normative tables (Means and Standard Deviations) on the ImPACT test for the five participant groups at each of the five test intervals, including data for: the five composite scores (Verbal Memory, Visual Memory, Visual Motor Speed, Reaction Time, Impulse Control); for the twelve subtest scores test that go to make up the composite scores; and for four additional memory subcomponent scores (Word Memory immediate recall, Word Memory delayed recall, Design Memory immediate recall, Design Memory delayed recall). The results provide a clinical and heuristic normative platform for future use with brain injured individuals, which can be used to facilitate clinical interpretations of postconcussion assessments.

Independent and dependent statistical analyses were employed to compare the rugby versus control group neurocognitive test profiles at and between the three test intervals. Correlational analyses explored the association between concussion, tackling and neurocognitive test outcomes. Descriptive comparisons of individual neurocognitive test scores with normative data were employed for the case analyses. Taken together, the results implicated vulnerability amongst club rugby players on the motor and speeded tasks, with less robust indications on the memory tasks. While limited in terms of its small sample size, it is considered that the outcome of the study was rendered more robust by virtue of being methodologically multifaceted with heuristic implications for future research studies in the area. The novel inclusion of tackling data as well as fine-tuned case analyses, were of particular relevance in that regard. The results add to a growing body of literature that implicates deleterious neurocognitive effects in participants of a sport such as rugby due to repetitive head jarring incidents that are intrinsic to the game.

ImPACT, a computer-based neuropsychological assessment instrument, has been recognised as a valid and reliable tool in the monitoring of athletes' symptoms and neurocognitive functioning preseason and postconcussion. As a part of larger-scale concussion research conducted on top-team university rugby players, this is an in-depth case study conducted on a 20-year old participant with a history of multiple concussions, who was referred following a concussion sustained during the season. The objectives of the study were: (i) to determine the sensitivity of ImPACT versus WAIS-III Digit Span and Trail Making Test during the acute postconcussive phase; and (ii) to examine the sensitivity of ImPACT versus a comprehensive battery of neuropsychological tests to possible residual deficits as a result of the multiple concussions. ImPACT was determined to be more sensitive to acute postconcussive impairment following concussion than Digit Span and Trail Making Test. Furthermore, the ImPACT preseason baseline scores appear to be sensitive to neurocognitive dysfunction, possibly due to cumulative concussive injuries.

ImPACT is a valid and reliable measure of a variety of cognitive functions commonly affected by such injuries, which allows for objective return-to-play decision making (Iverson, Lovell, & Collins, 2003). However, studies show that the transfer of such tests from one ethnic group to another without appropriate standardization is highly problematic (Ardila, 1995). Thus, the relative absence of South African normative data for the ImPACT 3.0 programme is an issue for concern. Consequently, this study aimed to establish norms for semi-rural Xhosa-speaking schoolboys with an advantaged education (ages 14, 16 and 18) for the ImPACT 3.0 programme as administered in English. Administrative and linguistic difficulties that were experienced by individuals during completion of the battery were also identified. Finally, the study included a comparison of the percentile scores of this sample to the USA norms for boys of a similar age group. Subtests scores were generated for 70 schoolboys and the data were then subjected to statistical analysis. A significant difference between English proficiency of the Grade 8 and Grade 12 boys was found. This indicates the importance of including an English proficiency test with the ImPACT battery when assessing such populations. No other significant differences were found between these age group samples. Although this requires further investigation, the comparison of the USA and SA percentiles suggests the use of local norms for this population. South African boys in this study consistently scored lower than the USA sample. Finally, it is recommended that symptom selfreports should be verbally investigated with each boy after testing, given indications of comprehension problems. In closing, limitations and future possible studies are discussed.

This in itself is quite a cumbersome process, thus the present study set out to ascertain whether baseline testing of all athletes is necessary, or whether the use of US or SA normative data alone would provide an appropriate standard against which to interpret the postinjury scores. From a leading South African rugby playing school, the 1st and 2nd rugby teams, (16 - 18 years) were baselined using ImPACT. Three athletes, who were subsequently referred with concussion during the rugby season, were followed up with serial testing on ImPACT. An analysis of the follow up scores was conducted to chart the athletes' recovery process, in relation to the athletes own baseline scores (using US and SA reliable change indices) and age appropriate US and SA normative ranges. The relative utility of individual baselines scores versus these normative ranges was then critically evaluated. It was concluded that a combination of both baseline and normative data provided optimal management of the athlete, with the methods complementing each other in the interpretation of post-injury results. Overall, the SA normative ranges seemed to provide slightly better management guidelines than the US normative ranges when used with this sample of South African high school athletes.

This study examined the effects of age and education on the Trail Making Test, the Digit Symbol Sub-Test, and the Finger Tapping Test in a non-clinical sample of community dwellers with a relatively low level of education (8 to 12 years) in South Africa. The sample consisted of 161 participants across six age groups: 20-39, 40-59, 69-69, 70-79, 80-89 and 90-95 years. Results were examined for mean age effects and variability trends. Highly significant age effects were present across the age groups for all tests, however, there was no uniform pattern of variability across the tests. The Digit Symbol Substitution Test and the Finger Tapping Tests showed a pattern of increasing variability with increasing age, followed by a decrease in very old age while no trend was evident for the Digit Symbol extensions (the Immediate and Delayed Recall tests). The Trail Making Test, Parts A and B, showed a consistent trend of increasing variability across the age groups. Data from the present study was compared with existing data from two relatively high education samples, with equivalent age groupings, to examine education effects. Results showed an education effect for all tests with the high education groups outperforming the low education groups. Although the effects of education became less potent with advancing age, the mean performance of the oldest (80-89 years) high education age group was superior to that of the equivalent low education age group. Comparison of variability trends across both samples showed that the highest variability (the shuttle bulge) was present at the same point along the age axis, or at a later point, for the low education group, as that for the high education group. This finding is inconsistent with Jordan's (1997) 'shuttle model of variability' which predicts an earlier occurrence of the shuttle bulge (left shuttle shift effect) for a low education sample. This study demonstrated that performance on neuropsychological tests is influenced by age and education and highlighted the dangers inherent in unquestionably applying norms, which have not been corrected for age and education, when assessing the older adult.

This study demonstrated that performance on neuropsychological tests is influenced by age and education and highlighted the dangers inherent in unquestionably applying norms, which have not been corrected for age and education, when assessing the older adult.

A comprehensive battery of neuropsychological tests was administered to top national (Springbok) rugby players (n=26), national Under 21 rugby players (n= 19), and a non-contact sport control group of national hockey players (n=21). The test results of the Total Rugby group (Springbok Rugby and Under 21 Rugby players), the Under 21 Rugby group, the hockey controls, and the Total Rugby and Under 21 Rugby forward and backline players respectively, were each compared with established normative data. Results showed significant differences in the direction of a poorer performance relative to the norms for the Total Rugby and Under 21 Rugby groups, and for the Total Rugby Forwards and Under 21 Rugby Forwards, on tests sensitive to the effects of diffuse brain damage. On the other hand, the Hockey Control group and the Total Rugby Backs and Under 21 Rugby Backs tended to perform within the normal range or better than the norm on some tests. These results confirm the hypothesis that rugby players, and the forward players in particular, are at risk of adverse cognitive effects consequent on cumulative mild head injury. The theoretical implications are that the aggregate effects of multiple exposures to mild head injuries in the rugby players served to reduce their brain reserve capacities and acted as a threshold-lowering influence associated with symptom onset.

Mean score comparisons within the rugby group indicated that it was the subgroup of forward players, in particular, whose test performances revealed deficits suggestive of cerebral damage. Specifically, deficits were found in working memory, visuoperceptual tracking, verbal memory and visual memory, a pattern of deficits commensurate with cumulative mild head injury. The theoretical perspectives of Satz’s (1997) Brain Reserve Capacity Theory and Jordan’s (1997) ‘Shuttle’ model of variability are drawn upon in order to elucidate research findings and suggestions for future research are provided.
